Country to Coast Queensland (CCQ) delivers the Australian Government's Primary Health Network (PHN) program for Central Queensland, Wide Bay and Sunshine Coast. As a not-for-profit organisation, we work with communities, clinicians and partners to strengthen primary care and help people live their healthiest lives.
What we do:
• Coordinate primary health services to meet local needs
• Improve the patient journey between primary and acute care
• Support general practice and the broader primary care workforce
• Improve access to services in rural and remote communities
• Reduce pressure on hospitals by strengthening community-based care
Our events bring together health professionals, community leaders and service providers to share insights, build partnerships and stay equipped to meet the evolving health needs of our region.
